The National Transportation Safety Board’s final report on the 2022 helicopter crash that killed a pilot and a Charlotte TV meteorologist revealed disconnected and missing hardware as the probable cause.

The crash occurred during a training flight over a Charlotte-area interstate, with the helicopter losing control and crashing in a grassy area.

The report highlighted inadequate inspections by the pilot and maintenance personnel, leading to the loosening of critical hardware.

A lawsuit has been filed by the meteorologist’s wife, alleging negligence on the part of the maintenance facility and parent companies involved.

Aviation attorney Gary C. Robb emphasized the need for improved safety measures in light of the maintenance errors uncovered in the investigation.

Pilot Chip Tayag and WBTV meteoroligist Jason Myers died in the crash in Nov. 2022.
